Who can attend
All of these need to apply.
1 · 1. Must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
2 · 2. Must live in one of the following areas
- Ararat
- East Grampians
3 · 3. Must have a referral from one of the following
- Hospital or community-based health professional
- General Practitioner (GP)
- Self-referral or referral from family members are accepted
The cost of the initial assessment is $10.50 or $15.50 (based on individual circumstances). The cost to attend the Falls and Balance Program is $50.
