More information
Eligibility heading 3 ("Must have a referral from one of the following:") is split on the page into two sub-groups: "Integrated Falls Assessment Service" (Hospital or community-based medical professional; General Practitioner (GP)) and "No Falls Exercise Program" (Hospital or community-based health professional; General Practitioner (GP); Self-referral or referral from family members are accepted). The schema has no field for eligibility sub-groups, so the sub-group headings are retained inline within the items list.
