Who can attend
All of these need to apply.
1 · Must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
2 · Must live in one of the following areas
- No catchment area
3 · Must have a referral from one of the following
- Hospital or community-based health professional
- General Practitioner (GP)
- My Aged Care – For all individuals aged 65 and over (50 years or older for Aboriginal and Torres Strait Islander)
- Self-referral and referral from family members are accepted
The cost to attend the Falls and Balance Program is $5 per session.
