Who can attend
All of these need to apply.
1 · Must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
2 · Must live in one of the following areas
- City of Boroondara
- City of Yarra
- City of Darebin (Alphington, Fairfield, Northcote, Thornbury, Westgarth)
3 · Must have a referral from one of the following
- Hospital or community-based health professional
- General Practitioner (GP)
- Self-referral or referral from family members are accepted
There is no cost to attend the Falls and Balance Assessment Clinic. The cost to attend the Falls and Balance Program is $7 per session.
