1 · must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
2 · must live in one of the following areas
- Mount Alexander Shire
- Central Goldfields Shire
- Macedon Ranges Shire
3 · must be referred by
- Hospital or community-based health professional
- General Practitioner (GP)
- Self-referral or referral from family members are accepted
