Who can attend
All of these need to apply.
1 · Must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
- Falls where the cause is complex or unclear
2 · Must live in one of the following areas
- Shire of the Yarra Ranges
- The suburbs of Croydon Hills, Croydon North and Croydon South in the City of Maroondah
3 · Must have a referral from one of the following
- Hospital or community-based health professional
- General Practitioner (GP)
- Self-referral or referral from family members are accepted
$10 co-payment per face-to-face visit
