Recent fall
History of falls
Risk of falls
Fear of falls

2 · Must live or work in one of the following areas

City of Wyndham Vale

3 · Must have a referral from one of the following

Hospital or community-based health professional
General Practitioner (GP)
Self-referral or referral from family members are accepted
Must have a referral from MAC if >65 years old
