Who can attend
All of these need to apply.
1 · 1. Must have one (or more) of the following health problems
- Recent fall
- History of falls
- Risk of falls
- Fear of falls
2 · 2. Must live in one of the following areas
- Foster and neighbouring towns (such as Stony Creek, Fish Creek, Walkville, Welshpool)
3 · 3. Must have a referral from one of the following
- Community-based health professional (Aged Care Assessment Service)
- General Practitioner (GP)
$80 for Commonwealth Home Support Program (CHSP) clients per program.
